The survey is completed online, using any Internet browser (Internet Explorer, Firefox, Chrome, etc.). The survey software uses the highest-quality security practices available, and is verified for use by the US Government. This survey WILL NOT
- Divulge any proprietary and/or confidential information to anyone other than the MVHCP program staff and limited Government and Military personnel
- Result in attempts to sell you or your company any products, technologies or information.
Instructions
To begin the survey, please go to the survey URL. Information about the survey is given before questions are asked. The only required questions are those asking for contact information for the company and the primary point of contact;however, the more complete the survey answers, themore useful they are.
- Please use the "Next" and "Back" buttons at the bottom of each page of the survey to navigate. The “Back” button on the survey only goes through a few pages.
- Please avoid using the "forward" and "back" buttons on your browser as those do not control navigation in the survey.
- To return to the survey at a later time you may click the “Save” button and copy the URL that appears. When you wish to complete the survey, go to that URL. Unfortunately you will not be able to return to previously-completed pages.
- Underlined words in the survey are defined – hold your mouse cursor over these words to see their meanings.
- For records, you may print individual pages as you complete them, take notes, or choose to see a summary of your responses at the completion of the survey.
- If you would like to make changes to your survey responses after submission, please contact Ms. Margaret Eckman at (919) 969-8008 or Eckman@idb.org.

Completing the Survey
If you have started the survey and need to complete it, please use the following options. Please contact Ms. Margaret Eckman at (919) 969-8008 or Eckman@idb.org with any questions.
- If you hit “Save” on the survey the last time and saved the custom URL that appeared, use it.
- You can try to use the same computer and browser as the last time you did it. (This option is hit-or-miss.)
- You can fill in the contact information again (required so we can keep information straight) and skip the other questions you have already answered.
- You can fill out the survey in its entirety.
Collateral
If you have taken the survey and wish to send us any collateral, such as brochures or test data, please send it! We now have the ability to attach it to your company profile. You can email it to Eckman@idb.org (preferred) or mail it Attn: Margaret Eckman, Institute for Defense and Business, 1430 Environ Way, Chapel Hill, NC 27517.
